Promoting wellness in our community through preventative healthcare is important because it can help to reduce the incidence of chronic diseases, such as heart disease, stroke, cancer and diabetes. It can also help to improve the quality of life for those who are living with chronic illnesses. Furthermore, preventative healthcare can help to reduce healthcare costs by preventing or delaying the onset of chronic diseases. Health promotion and disease prevention are essential components of a healthy community. Wellness programs that promote healthy lifestyles and provide early detection and treatment of health problems can make a positive impact on the health of our community.

Healthy Kids Club

#HealthyKids

The Healthy Kids Club (HKC) is a program focused on promoting better health outcomes for children. The program emphasizes the need to stay immunized, keeping up with medical, eye, and dental well checks, and encourages behavioral health visits when patients are struggling with emotional or mental health issues. The program supports these families by recognizing when they attend their visits and is available to children ages birth to fifteen years.

Empowering Women's Wellness: Support Our Women's Health Initiative

Pregnancy is an exciting time, often filled with anticipation and hope. Our prenatal specialists help local parents and their babies stay healthy by encouraging early entry to prenatal care. This allows for appropriate education and support hoping to reduce infant mortality or low birth weight babies.

How can you help? Help fund our OB-GYNs, family medicine physicians, and certified nurse midwives care for these mothers and babies during their pregnancy.

Ms. Susie Froese searched for a midwife in a hospital setting, she was recommended to Stephanie White, MSN, RNC-OB, CNM.

Building a Healthier West Texas Capital Campaign

Community Health Center moved into our new building in December 2015 in the downtown area. Through many generous donors, we have raised 72.94% of the goal of raising $14.5 million dollars. When you give to our Capital Campaign, you help to free up our operating revenue to provide for more quality patient care. Lubbock needs your help to build a healthier West Texas. In 1992, Sister Mary Kathleen Small laid the foundation for Community Health Center of Lubbock, Inc. (CHCL), the area’s leading provider of low-cost quality primary and preventative health care services.

Are you funded by the City of Lubbock or the State of Texas?

We are a nonprofit organization and rely on gifts from individuals, corporations and foundations.

What types of gifts do you accept?

· Gifts of Cash – outright or pledged over a period of five years.

· Gifts of Securities – including stocks, bonds, mutual funds and IRAs.

· Gifts of Personal Property – i.e., coin collections, antique cards, artwork, etc.

· Gifts of Real Property – including real estate, home, business property, farmland and pastures, etc. with CHCL reserving the right to liquidate at any time.

· Gifts of Trusts – including charitable remainder trusts, unitrusts, lead trusts, family trusts, annuity trusts, living trusts, etc.

· Gifts of Insurance – CHCL must be both the owner and beneficiary for the donor to receive a charitable deduction.

· Gifts of Bequests – i.e. gifts received at death directed by a will or living trust.

· are inconsistent with our stated mission, purpose, and priorities

· violate the terms of our corporate charter

· could result in our violating federal or state laws

· are unduly burdensome to administer

· pose an unacceptable risk of liability to us

Yes. You can direct your gift to any department, location or program by indicating so on your online giving form.

Can I make a gift in honor of or in memory of someone?

Yes. You can make a gift to honor or remember an individual by directing your gift to the appropriate program, department or location. If you make your gift by mail, you can inform us by including a note in the envelope naming the honoree. After you have made your gift, if you wish, we will send an acknowledgement to that individual or their family.

Is my gift tax-deductible?

Community Health Center of Lubbock is a nonprofit 501©(3) charitable organization, and contributions are tax-deductible to the extent allowed by law. Our tax identification number is 75-2424925.

To confirm that we have received your gift and for tax purposes, you will receive a letter documenting the gift amount.
