We understand that forms and paperwork (such as FMLA, disability or other documentation) are important for your care and daily life. To help set expectations, please review the following information:
Turnaround Time
Paperwork is typically completed within up to 10 business days after it has been received by our office.
Some forms may take longer if they require additional information from your provider or outside records.
What You Can Do
Submit forms early – bring the paperwork as soon as you receive it.
Complete your section first – incomplete forms may delay processing.
Provide contact details – let us know where to send the paperwork or if you’ll pick it up.
Appointment Requirement
An appointment may be required by your provider to review the paperwork with you.
If paperwork cannot be completed during the appointment, staff will notify you once it is ready.
Urgent Requests
Please inform staff if you need paperwork completed sooner than the standard turnaround time.
We will do our best to accommodate urgent requests based on provider availability.
Same-day or next-day completion may not be possible.
Notification
You will be contacted by our office once your paperwork is complete.
Completed paperwork will be scanned into your chart and can be accessed in the patient portal (MyChart) under the “Letters” or “Documents” section for easy viewing or downloading.
